Furthermore, the width of the graph (shaded area) is wide during these times, indicating significant day-to-day variability in blood glucose. Whole wheat bread is generally considered "rich in dietary fiber and less likely to raise blood sugar." However, my Continuous Glucose Monitor (CGM) captured a shocking graph of a rapid surge, with a peak blood sugar of 220 mg/dL. I thought I was choosing a healthy food, so why did it cause such a spike? Hidden behind this phenomenon was a subtle "time lag" between the action time of rapid-acting insulin and the timing of my meal. Let's logically examine this based on actual data.

1. Tracing the Day's Timeline and Overall Statistics from the Log

First, let's organize the menu I consumed, the timing of insulin administration, and the overall statistics obtained for this experiment. What appeared to be ideally controlled morning blood sugar levels would drastically change just a few hours later.

06:30 Administered 8 units of rapid-acting insulin "Humalog," then had breakfast (200ml milk, 1 egg, banana yogurt).
08:00 Reached the first peak (approx. 205 mg/dL). Afterward, blood sugar steadily decreased due to the insulin's effect.
09:30 Blood sugar dropped steadily to 97 mg/dL. At this time, I consumed a single slice of whole wheat bread (equivalent to one 8-slice portion) baked in a home bakery, by itself.
11:00 Recorded the second peak (highest value 220 mg/dL).

2. Blood Sugar Graph Analysis: Why the "Diabolical M-Shape" Formed

The CGM graph recorded clearly shows two distinct peaks, forming what's known as an "M-shaped pattern." What's noteworthy here is that the second peak, after eating just one slice of supposedly healthy whole wheat bread, is clearly higher and sharper than the first peak, which occurred after consuming dairy and fruit.

The First Peak (06:30-09:30): Precise Insulin Action

Due to the lactose in dairy products (milk, yogurt) and fructose in the banana, my blood sugar temporarily rose to approximately 205 mg/dL. However, the 8 units of Humalog administered at 06:30 took full effect, bringing the blood sugar down smartly to an extremely ideal baseline of 97 mg/dL by 09:30. Up to this point, it was perfect control.

The Second Peak (09:30-12:00): Carbohydrate Intake in a Vulnerable State

The problem began immediately after I consumed the freshly baked whole wheat bread at 09:30, just when my blood sugar had settled. As the yellow shaded zone on the graph shows, the number recorded a rocket-like surge of +123 mg/dL in just an hour and a half, skyrocketing past 220 mg/dL.

3. The Clinical Perspective: The Insulin "Blind Spot"

There's no doubt that whole wheat has a lower Glycemic Index (GI) and is absorbed more slowly than refined white rice or white flour. Nevertheless, the cause of such a significant spike was not the quality of the food itself, but rather a "gap in the body's insulin dynamics."

Rapid-acting insulin preparations like "Humalog," which I used, begin to act approximately 15 minutes after administration, peak within 1 to 3 hours, and then lose their effect within about 3 to 5 hours.

The insulin administered at 06:30 was already in the final stages of its activity by 09:30, three hours later. In other words, my body was in a "vulnerable state with almost no insulin remaining to process additional carbohydrates." When I introduced bread, which is still a carbohydrate despite being low GI, the body couldn't process the sugar, and it directly flooded my bloodstream, reflecting in the drastic numbers.

■ Key Takeaway from This Experiment

It's the realization that "just because current blood sugar is low (97 mg/dL) doesn't mean it's safe to eat additional carbohydrates." What's crucial isn't merely the current number, but rather the timeline perspective: "how much active insulin remains in the body to handle the carbohydrates you are about to consume."

4. Future Measures and Conclusion

It's not easy to maintain composure in front of the wonderful aroma of freshly baked bread wafting from a home bakery. If I were to split my staple food intake (as a snack) with a time delay, as I did this time, I reflect that the following approaches would have been necessary:

1. Reduce the 06:30 insulin dose to match breakfast only, and "split-dose" a small amount of additional insulin to coincide with the 09:30 bread intake.
2. Alternatively, delay bread consumption until the next meal (lunch) to avoid needing additional insulin.

The biggest advantage of CGM is its ability to visualize the "wave of insulin's effectiveness" in your body. This experiment affirmed my belief that mastering this "time-axis control," in addition to knowing how to choose quality ingredients, is the key to more refined blood sugar management.

  • However, hypoglycemia (below 70mg/dL) is almost nonexistent, and **stable avoidance of hypoglycemia** is excellent. Since hypoglycemia can sometimes be dangerous, this is a great relief.
  • On some days, for example, from the night of May 24th to the morning of May 25th, and during the daytime of May 29th, **many periods within the target range are observed**, indicating relatively stable blood glucose levels. This suggests that the diet and activities on those days might have been good, providing hints for replicating good states.

From this graph, it seems that the main goal for the next step will be "how to reduce the periods of high blood glucose."

2. Check Daily Summaries for Day-Specific Trends!

Next, let's examine the average blood glucose value for each day and the daily blood glucose patterns over 24 hours. We will analyze in detail what differences exist depending on the day of the week.

  • Particularly Good Day: Friday (May 29th)
    • On this day, the average blood glucose was 145mg/dL, the lowest for this week. Looking at the graph, blood glucose levels from morning to evening were relatively stable, mostly remaining within the target range. Even after dinner, the rise was gentle, which is excellent control! The way you spent this day and your meal content might hold clues for the future.
  • Days Showing Hints for Improvement: Tuesday, Wednesday, Thursday, Saturday (May 26th, 27th, 28th, 30th)
    • On these days, average blood glucose levels were high (235mg/dL to 270mg/dL), with high blood glucose exceeding 200mg/dL for many hours during the day. Especially, sharp rises in blood glucose after breakfast and dinner, followed by sustained high levels, are notable. A higher tendency is also seen at night.
    • Monday (May 25th) and Sunday (May 24th) also had slightly higher averages, but were within the target range during some periods.

  • Relatively Stable Periods: From around 2 PM to 5 PM, the median remains at a relatively low position, indicating periods where blood glucose levels are settled.

This graph tells us that managing blood glucose, especially from nighttime to morning and after dinner, is crucial. A large fluctuation range also suggests variability in meal content and activity levels.
