I still remember the sheer panic that washed over me when the first blood test results came back after I was rushed to the ER. Even the doctors were stunned by the off-the-charts numbers. Let’s take a look at the terrifying, life-threatening data from the day I was admitted.

[The Shocking Blood Sugar Levels on Admission Day]

  • When I was first brought in, my blood glucose level was at a mind-boggling 828 mg/dL.
  • This was astronomical compared to the normal range of 70–109 mg/dL. It was a miracle I hadn't completely slipped into a coma.

  • My CPK levels, which skyrocket when there is massive muscle breakdown, had shot up to a staggering 11806 U/L.
  • Because of the sheer amount of waste from the damaged muscle fibers, my kidneys were struggling to cope. My blood urea nitrogen (BUN) rose to 63 mg/dL and creatinine hit 2.03 mg/dL—my kidneys were practically screaming for help.
  • To make matters worse, my CA19-9 level (an indicator of severe pancreatic inflammation) surged to 401.0 U/mL. It felt like multiple massive fires were raging inside my body all at the same time.

[Surviving the ICU: My Memories of Those Critical Days]

  • Fading consciousness in the ambulance: I have a hazy memory of being loaded onto the stretcher and put into the ambulance, but after that, everything goes completely blank. My wife later told me that my speech was so slurred that the paramedics asked her, "Does your husband always talk like this?"
  • Waking up to a web of wires: When I finally opened my eyes, I was in a hospital bed. I only found out later that it was the ICU. My arms were covered in a dizzying array of IV lines and blood monitors.
  • Delusional work anxiety: Apparently, in my confused state, I kept babbling to the nurses about needing to get to my job because I'm a designer, only to be gently told to calm down.
  • Hallucinations of a computer screen: There was a small, somewhat bright window in the room. Every time I closed my eyes, that square shape would expand and transform into a computer monitor, showing a desktop screen. I kept looping between opening my eyes to see the hospital room and closing them to see this hallucination.
  • Vision issues caused by high blood sugar: Once my consciousness returned, I tried putting on my glasses, but everything was incredibly blurry. An ophthalmologist examined my eyes but found nothing physically wrong. For a while after that, my ability to see far-off objects would fluctuate constantly depending on my blood sugar levels.
  • The harsh reality of the ICU: The patient next to me was coughing constantly and needed regular phlegm suctioning. I also remember hearing hospital staff discussing funeral arrangements nearby on my second day, realizing someone had passed away. It was a sobering environment.
  • Transfer to the general ward on Day 3: After about three days, my blood work stabilized enough for me to be moved out of the ICU and into a general ward.

Since you're piercing your skin every day, many of you are probably thinking, "I want to reduce the pain as much as possible" or "I want to find the needle that's right for me," right? In this post, I'll be reviewing two types of injection needles I personally use: Micro-Fine Pro 32G and Nanopass Needle II 34G. I'll cover their gauge, sharpness, and gentle feel.

Understanding Insulin Needle "Gauge" and "Sharpness (Bevel Design)"

When choosing an insulin injection needle, a crucial indicator is the "G (gauge)" unit. The larger the G number, the finer the needle's diameter becomes.
It's commonly believed that "the finer the needle, the less painful it is," but in reality, the needle's "sharpness (the bevel design of the tip)" also plays a significant role in pain reduction.

【Review】My Experience and How to Choose the Right Needle for You

While needle compatibility certainly varies from person to person, to put it simply, for me, the "green needle (Micro-Fine Pro 32G)" was a better fit.

Differences in "Ease of Insertion" When Injecting into the Abdomen

As part of my daily routine, I primarily inject into my abdomen. I've found that the green needle, thanks to its sharp tip, penetrates the abdominal skin smoothly without snagging.

On the other hand, while the orange needle (34G) benefits from being ultra-fine, some people might experience a slight resistance or a bending sensation when it penetrates the skin.

How to Avoid "Nerve-Hitting Pain"

However, the green needle does have a drawback. If you're unlucky enough to hit a nerve beneath the abdominal skin, you might feel a sharp, stinging pain.
In that regard, the orange needle, being ultra-fine, seems to have a lower probability of stimulating nerves. Therefore, I recommend choosing based on the following criteria:

Hello everyone! Today, I'm reporting on a day of intense blood sugar fluctuations, as recorded on my CGM (Continuous Glucose Monitor) graph.

I'll be looking back at a day where a moment of complacency – thinking 'I walked a lot, so I'll be fine!' – combined with insulin timing and the actual meal choices, spectacularly led to a severe blood sugar spike followed by a roller-coaster ride.

A Day's Blood Sugar Graph (CGM Data)

Timeline of Activities, Meals, and Insulin on This Day

  • 10:00 AM ~ 3:00 PM Long walk in the Omotesando-Roppongi area (a solid 5 hours of walking)
  • Around 12:00 PM (Lunch) Udon + Shirasu-don at Udon Shiki Roppongi Before meal: Humalog 12 units
  • 3:00 PM (Snack) "Kürtősh" + Coffee at Balaton Cafe Omotesando
  • Around 9:00 PM (Dinner) Low-carb meal (hot pot vegetables and pork) Before meal: Humalog 12 units + Tresiba 7 units
  • Around 10:00 PM Blood sugar plummeted to around 60 mg/dL (entering hypoglycemic range)
  • 10:30 PM (Correction Snack) Consumed Ramune, half a Dekopon orange, and 2 strawberries ➔ Successfully recovered afterwards

What Happened? Analyzing the Blood Sugar Rollercoaster Mechanism

Cause ①: The Rocket Surge between 3:00 PM and 4:00 PM

The 5-hour walk depleted my body's sugar energy, leaving my cells in a 'starved state,' intensely craving glucose. Into this state, I rapidly consumed a 'Kürtősh' — packed with sugar and flour, and incredibly fast-absorbing — which caused a ferocious blood sugar spike, shooting up to nearly 300 mg/dL.

Cause ②: The Plunge to the Abyss around 10:00 PM

I administered insulin before dinner, but the side dish was a 'hot pot' – an extremely low-carb meal. This meant there wasn't enough carbohydrate to counteract the insulin. Furthermore, the residual effect of the 5-hour walk made the insulin several times more effective than usual, causing my blood sugar to plummet abruptly to 60 mg/dL.

A Fine Play Against Hypoglycemia: My Correction Snack Choice

The Divine Relay: "Ramune + Half a Dekopon ➔ 2 Strawberries"

As my blood sugar dropped to around 60 mg/dL and symptoms appeared, I used fast-acting Ramune (glucose) for an emergency rescue. Afterwards, to prevent a 'second dip' from lingering insulin effects, I sustained sugar absorption gradually with the water and fiber from the fruits. My blood sugar then maintained a nice flat line around 80-90 mg/dL!

Future Strategies and Lessons Learned (Summary)

  • Avoid the 'first bite' of sweets after prolonged exercise: Have coffee as a latte, or eat some nuts or protein before indulging in Kürtősh.
  • Balance dinner menu and insulin on exercise days: If cutting carbs and only eating hot pot, reduce insulin units after consulting your doctor. Alternatively, since it's after exercise, intentionally eat a moderate amount of rice to balance it out.
  • Share data with your doctor: Discuss this valuable pattern data – 'long exercise + no-carb dinner + usual insulin dose = hypoglycemia' – at your next appointment.

Hello everyone! Today, I'm reporting on a day of intense blood sugar fluctuations, as captured by my CGM (Continuous Glucose Monitor) graph.

I'll look back at a day where my overconfidence of "I walked a lot, so I'll be fine!", combined with insulin timing and the actual meal choices, intricately conspired to create a spectacular blood sugar spike followed by a true blood sugar rollercoaster.

Timeline of Actions, Meals, and Insulin on This Day

  • 10:00 〜 15:00 Long walk through Omotesando ~ Roppongi area (a solid 5 hours of walking)
  • Around 12:00 (Lunch) Udon + Shirasu Don (Whitebait Bowl) at Udon Shiki Roppongi Pre-meal: Humalog 12 units
  • 15:00 (Snack) Kurtosh + Coffee at Balaton Cafe in Omotesando
  • Around 21:00 (Dinner) Low-carb meal (hot pot vegetables and pork) Pre-meal: Humalog 12 units + Tresiba 7 units
  • Around 22:00 Blood sugar plummeted to around 60 mg/dL (entering the hypoglycemia zone)
  • 22:30 (Rescue Snack) Consumed Ramune, half a Dekopon (tangerine), and 2 strawberries ➔ Successfully recovered afterwards

Why Did It Happen? Analyzing the Blood Sugar Rollercoaster Mechanism

Cause ①: Rocketing Spike from 15:00 to 16:00

The 5-hour walk depleted my body's glucose energy, leaving my cells in a "starved state" craving sugar. Pouring in "Kurtosh" – packed with sugar and flour, and absorbed at an ultra-fast pace – caused a furious blood sugar spike, shooting up to nearly 300 mg/dL.

Cause ②: Plunge to the Abyss Around 22:00

I injected insulin before dinner, but since the side dish was a "hot pot" – an extremely low-carb meal – there was a critical lack of carbs to offset the insulin. Furthermore, the delayed effect of the 5-hour walk made the insulin several times more potent than usual, leading to a sudden drop to 60 mg/dL.

A Fine Play Against Hypoglycemia: My Rescue Snack Choices This Time

The Divine Relay: Ramune + Half Dekopon ➔ 2 Strawberries

As my blood sugar dropped to around 60 mg/dL and symptoms appeared, I initiated an emergency rescue with fast-acting Ramune (glucose tablets). Afterward, to prevent a "second dip" from the lingering insulin effect, I used the water and fiber from fruits to sustain a gradual sugar absorption. After the rescue snacks, my blood sugar nicely maintained a flat line around 80-90 mg/dL!

Future Strategies and Lessons Learned (Summary)

  • Avoid the "first bite" of sweets after prolonged exercise: Have coffee as a latte, or eat some nuts or protein before the Kurtosh.
  • Balance dinner menu and insulin on exercise days: If cutting carbs and only eating hot pot, discuss with your doctor to reduce insulin units. Alternatively, since it's after exercise, intentionally eat a moderate amount of rice to balance it out.
  • Share data with your doctor: Discuss this valuable pattern data – "prolonged exercise + no-carb dinner + usual insulin units = hypoglycemia" – at your next appointment.

The 3 PM snack I had at Balaton Cafe in Omotesando was "Kurtosh," a traditional Hungarian pastry. It's a unique sweet characterized by its chimney-like shape, made by wrapping rope-like dough around a cylinder and baking it.
The freshly baked dough is generously coated with sugar and cinnamon, making it incredibly delicious. However, its main components are "flour + butter + plenty of sugar," resulting in a very high carbohydrate content that can cause a dramatic blood sugar spike when eaten alone.
